District Nurse Home Visits
The Community Health professional will arrange a time to visit patients in their home or see them at a clinic nearby. Our services cover:
- Assessment and care of complex wounds including specialist wound treatment.
- Continence assessment
- Stoma care - support, management and equipment to enable self care.
- Oncology care
- Home Help (short term)
- Meals on Wheels (short term)
- IV Therapy (short term)
- Assessment for short term personal cares while recovering from an acute illness
It is your responsibility to ensure safe access to your property, a safe environment and that dogs are tied up or locked away when the nurse visits. Please provide a smoke free atmosphere for our nurses.
Cancellations
Please notify us as soon as possible if you are unavailable for a visit or unable to attend a clinic appointment.
If you do not notify the office, the nurse will attempt to contact you one more time. If this is unsuccessful, you will be discharged to your GP's care.

Discharge Plan
You will be discharged when you have been assessed as no longer requiring our service. You will be advised of the final visit date and follow up care plan.
