HealthPathways is a tool designed to help general practices manage and refer patients to secondary, tertiary, and community services. The pathways are designed to be used by GPs and clinicians during consultation with the patient.

What is a care pathway?

In the context of healthcare, a care pathway describes the route a patient takes as they move through the health system and receive healthcare.

Who developed the HealthPathways?

General practice teams and hospital clinicians from the Wairarapa, Hutt Valley and Capital & Coast areas.

Will more pathways be added over time?

The 3D HealthPathways team are currently working on publishing additional new pathways developed across the subregion in recent years. As time allows, they will also work through the existing Canterbury pathways to reflect our local service arrangements.
